Fee Waiver has been issued for tribal ID request until further notice.<\/strong><\/p>\n\n\n\n

Listed below are instructions for applying for Tribal ID (online or visit to the Enrollment office)<\/p>\n\n\n\n

Please follow instructions if you choose to apply online.<\/u><\/strong><\/p>\n\n\n\n

 1. Require your photo<\/strong>. Please use your phone to take a selfie. Recommend taking a picture using a white as possible background, <\/em>with no objects<\/em><\/strong> in the background.<\/em><\/p>\n\n\n\n

2. Require your signature<\/strong>. Please sign your name on a blank white sheet of paper, if possible, use a sharpie marker (sometimes the signature are hard to see). Then with your phone take a picture of your signature.   Email both picture and your signature as an email attachment.<\/p>\n\n\n\n

3.Verification of Identification<\/strong>. What is acceptable is listed on the application. Please scan or take a picture and send it as an attachment.<\/p>\n\n\n\n

4. Payment methods<\/strong>: Money order or Debt\/credit card payment. For all Debt\/credit card payments please contact Phyllis Naranjo – PH: 435-234-1138, if you are unable to make connection it is recommended to email her with a contact number. IN OFFICE VISIT<\/strong>:<\/p>\n\n\n\n

1. Please, if possible, fill out the online application before visit the Enrollment Office. <\/p>\n\n\n\n

2. Bring some form of Identification for verification.<\/p>\n\n\n\n

3. Recommend notifying the Enrollment office when you will be arriving to take your picture.<\/p>\n\n\n\n
